Nurse Care Manager

Heidy has a Bachelor of Science degree in Nursing from Dominican College in New York and has been a Registered Nurse for over 20 years. She is fluent in both English and Spanish. Heidy has a broad range of nursing experience in both acute and home care settings and has particular expertise in community health and care coordination, where she has managed and coordinated complex health care needs and services for clients with acute and chronic illnesses. In her role as a clinical nurse coordinator, Heidy has considerable experience with the role of a health advocate and liaison across the care continuum among clients, families, medical providers and facilities. She has also worked as a concierge, private duty nurse caring for high profile clients and has delivered the highest level of professional nursing care with empathy and compassion. Heidy is a certified nurse care manager and in addition is PRI and screen certified.
