Anne C. Sansevero, RN, MA, GNP, CCM
Anne Sansevero is the founder and CEO of HealthSense LLC, an Aging Life Care management consulting practice. She is a master’s prepared geriatric nurse practitioner, and a seasoned nursing professional with over 30 years of experience in the field. Anne has a sub-specialty in neurological disorders and is well versed in all aspects of geriatric nursing. She has particular expertise with communication disorders relating to stroke and dementia and has developed a number of innovative nursing assessment tools and standards to improve the nursing care for frail elders. Anne is a member of the Aging Life Care Association® (ALCA) and Fellow of the Aging Life Care Leadership Academy. She is currently serving on the board of the ALCA, and is Vice President of the New York Chapter. In addition, Anne is a member of the American Academy of Nurse Practitioners and the Nurse Practitioners of New York. Anne is a registered nurse, certified care manager, and a master’s prepared geriatric nurse practitioner.
“Born in Ireland, I come from a strong medical family of physicians and nurses. I was naturally drawn to a career in health care where you can make a real difference in the lives of others. In the 1980s, I was recruited for a nursing career opportunity in the United States and fell in love with the energy and optimism that is part of this country’s cultural DNA. In the acute care work setting, I became very attuned to how stressful hospitalization can be for older, frail adults. I found my calling as a nurse practitioner and Aging Life Care Professional while advocating for the elderly and working to improve their quality of life and to avoid hospitalizations. I have a deep appreciation for the wisdom and resilience that I see in my older clients and feel grateful to be able to contribute to helping them and their families lead happier lives.”
Thais Winmill, MS, RN-BC, FNP-BC
Thais Winmill is an Aging Life Care Professional who has been working with HealthSense clients since 2013. Thais graduated from the University of Texas at Arlington in 2009, and is board certified in cardiovascular nursing. She completed her Master’s in Advanced Practiced Nursing as a Family Nurse Practioner at New York University in 2016. Thais is a member of the American Academy of Nurse Practitioners.
“I have always enjoyed working with older adults, especially in community settings. I was drawn to the field of care management because it allowed me to work with seniors in a unique and personal way. In this role, I am empowered to individualize care, always respecting each person’s unique qualities. As a family nurse practitioner, I am sensitive to aging across the lifespan and the importance of family and caregiver support systems to overall well-being. I am grateful to be able to provide care management services within this holistic framework.”
Debra Shore, RN, MPH
Debra Shore is an Nurse Care Manager who has been working with HealthSense clients since 2015. Debra has over 25 years of nursing and healthcare administration experience. Debra graduated from Boston University where she was awarded with the prestigious Scarlet Key Award and was also a James Martin Scholar, both recognizing her nursing excellence. She received her Master’s Degree from Columbia University School of Public Health. Debra has a strong critical care background in cardiac and pulmonary care and acute inpatient rehabilitation at some of the top medical facilities in New York including Memorial Sloan Kettering Cancer Center, NYU Langone Medical Center and Rusk Rehabilitation Institute. Her clinical experience also extends into the private practice arena of concierge medicine and home care.

Mary Jo Vetter, DNP, RN, AGPCNP-BC
Mary Jo is a board-certified adult and geriatric primary care nurse practitioner who has practiced throughout the metropolitan area for the past 20 years. Her clinical expertise is focused on enabling aging individuals to remain safely at home. As a professional committed to patient and family-centered care, Mary Jo provides skillful navigation support to her clients during transitions from hospital to home, coordinating treatment planning with multiple specialists and empowering them to make informed decisions about longer-term care preferences. She has demonstrated capability in palliative and end-of-life care and is a compassionate and knowledgeable clinician. Mary Jo remains up to date on the latest advances in healthcare options as member of the American Academy of Home Care Medicine and the National Organization of Nurse Practitioner Faculties.

Samantha Clifford, RN, BSN
Samantha Clifford received her Bachelor’s of Science degree in Nursing from Brigham Young University with honors. Her clinical experience includes emergency geriatric care and rapid response triage. Samantha is also an expert on stroke care and cardiac assessment and management. She has extensive experience in counseling and advising families on geriatric health, safety and wellness initiatives and holds an ACE fitness certification as a personal trainer.
“It was while caring for my aging grandparents that I became passionate about issues affecting the aging community. I gained a deep satisfaction advocating for their needs and ensuring that they aged comfortably and safely in their homes. As a nurse care manager, I enjoy facilitating professional collaboration and participating in improving client outcomes. My goal is always to help my clients achieve wellness and happiness in their care environments.”
