Anne C. Sansevero, RN, MA, GNP, CCM

Anne Sansevero is the founder and CEO of HealthSense LLC, an Aging Life Care management consulting practice. She is a master’s prepared geriatric nurse practitioner, and a seasoned nursing professional with over 30 years of experience in the field. Anne has a sub-specialty in neurological disorders and is well versed in all aspects of geriatric nursing. She has particular expertise with communication disorders relating to stroke and dementia and has developed a number of innovative nursing assessment tools and standards to improve the nursing care for frail elders. Anne is a member of the Aging Life Care Association® (ALCA) and Fellow of the Aging Life Care Leadership Academy. She is currently serving on the board of the ALCA, and is Vice President of the New York Chapter. In addition, Anne is a member of the American Academy of Nurse Practitioners and the Nurse Practitioners of New York. Anne is a registered nurse, certified care manager, and a master’s prepared geriatric nurse practitioner.

“Born in Ireland, I come from a strong medical family of physicians and nurses. I was naturally drawn to a career in health care where you can make a real difference in the lives of others. In the 1980s, I was recruited for a nursing career opportunity in the United States and fell in love with the energy and optimism that is part of this country’s cultural DNA. In the acute care work setting, I became very attuned to how stressful hospitalization can be for older, frail adults. I found my calling as a nurse practitioner and Aging Life Care Professional while advocating for the elderly and working to improve their quality of life and to avoid hospitalizations. I have a deep appreciation for the wisdom and resilience that I see in my older clients and feel grateful to be able to contribute to helping them and their families lead happier lives.” 

Care Team - Debra Shore

Debra Shore, RN, MPH

Debra Shore is an Nurse Care Manager who has been working with HealthSense clients since 2015. Debra has over 25 years of nursing and healthcare administration experience. Debra graduated from Boston University where she was awarded with the prestigious Scarlet Key Award and was also a James Martin Scholar, both recognizing her nursing excellence. She received her Master’s Degree from Columbia University School of Public Health. Debra has a strong critical care background in cardiac and pulmonary care and acute inpatient rehabilitation at some of the top medical facilities in New York including Memorial Sloan Kettering Cancer Center, NYU Langone Medical Center and  Rusk Rehabilitation Institute. Her clinical experience also extends into the private practice arena of concierge medicine and home care.

Mary Jo Vetter DNP, RN, AGPCNP-BCMary Jo Vetter, DNP, RN, AGPCNP-BC

Mary Jo is a board-certified adult and geriatric primary care nurse practitioner who has practiced throughout the metropolitan area for the past 20 years. Her clinical expertise is focused on enabling aging individuals to remain safely at home. As a professional committed to patient and family-centered care, Mary Jo provides skillful navigation support to her clients during transitions from hospital to home, coordinating treatment planning with multiple specialists and empowering them to make informed decisions about longer-term care preferences. She has demonstrated capability in palliative and end-of-life care and is a compassionate and knowledgeable clinician. Mary Jo remains up to date on the latest advances in healthcare options as member of the American Academy of Home Care Medicine and the National Organization of Nurse Practitioner Faculties.

Samantha Clifford, RN, BSN

Samantha Clifford received her Bachelor’s of Science degree in Nursing from Brigham Young University with honors. Her clinical experience includes emergency geriatric care and rapid response triage. Samantha is also an expert on stroke care and cardiac assessment and management. She has extensive experience in counseling and advising families on geriatric health, safety and wellness initiatives and holds an ACE fitness certification as a personal trainer.

“It was while caring for my aging grandparents that I became passionate about issues affecting the aging community. I gained a deep satisfaction advocating for their needs and ensuring that they aged comfortably and safely in their homes. As a nurse care manager, I enjoy facilitating professional collaboration and participating in improving client outcomes. My goal is always to help my clients achieve wellness and happiness in their care environments.”

Heidy DeLaOsa Heidy DeLaOsa, RN, BSN, CCM

Heidy DeLaOsa has a Bachelor’s of Science degree in Nursing from Dominican College in New York and has been a Registered Nurse for over 20 years. She is fluent in both English and Spanish. Heidy has a broad range of nursing experience in both acute and home care settings and has particular expertise in community health and care coordination, where she has managed and coordinated complex health care needs and services for clients with acute and chronic illnesses. In her role as a clinical nurse coordinator, Heidy has considerable experience with the role of a health advocate and liaison across the care continuum among clients, families, medical providers and facilities. She has also worked as a concierge, private duty nurse caring for high profile clients and has delivered the highest level of professional nursing care with empathy and compassion. Heidy is a certified nurse care manager and in addition is PRI and screen certified.  

“I have always enjoyed problem solving and improving the lives of my clients by finding creative ways to enhance care. In my work as a care manager, I derive enormous satisfaction from navigating complex health issues and advocating for my clients to ensure excellent outcomes to enrich their quality of life. I pride myself on keeping current with the intricacies of health insurance plans and policies and local community resources. I also enjoy my role as a nurse community educator, promoting positive health habits so that clients can age optimally.”

Hector Pou, LCSW-R; C-ASWCM

With more than 30 years’ experience as a social worker and administrator serving older adults and their families, Hector has extensive expertise in helping families navigate elder care. Hector has a deep understanding of Medicaid eligibility criteria, the Social Services system, pooled trusts for income shelter and a broad range of insurance plans including long term care and managed care policies. Hector earned a Bachelor’s of Arts degree from Lehman College, City University of New York and a Masters in Social Work from Fordham University. He is a member of the Academy of Certified Social Workers (ACSW) and is a Certified Social Worker Care Manager (C-ASWCM). Hector was honored by the New York State Office of Aging in 1990 for outstanding contributions to the elderly of New York. Hector is also a member of the Aging Life Care Association (ALCA).
